Outreach Programs, C2C and CAMPP
C2C and CAMPP
Connect 2 Care (C2C) and Community Allied Mobile Palliative Partnership (CAMPP) are intensive case management teams providing transitional support from acute care to the community for low-income and socially vulnerable patients.
These teams provide navigational support to individuals to improve access to primary health care, mental health care, addiction support, housing, social and financial support in the community, and palliative and end-of-life care.

Reasons for referral could include (but are not limited to):
Connection to primary care
Frequent acute care use
Discharge planning/coordination
Health system navigation support
Addiction/detox/treatment support
Housing support
Mental health support
Palliative/end of life care (provided in partnership with CAMPP)

Other Outreach Programs
If you can't come to CUPS, you can still get medical care. Get outreach health care at six Calgary shelters.
You can still access direct health care from CUPS providers onsite if you are a client at one of the below agencies. If you’re a client at one of these agencies, connect with staff to learn more.

What do I need to have?
If you have an Alberta Health Care Card, please have it ready. If you don’t have one, you may need to have proof of low income or immigration documentation, depending on the Agency providing the services.
