KEY CASE MANAGEMENT
CUPS Key Case management focuses on maintaining long-term housing stability and improved well-being (physical and mental health). The goal is independence and community re-integration.
Our Case Managers support up to 120 adults who have experienced chronic or episodic homelessness. It includes:
access to permanent housing with no barriers or housing readiness (subsidy provided)
developing individualized service plans
enhancing life skills and addressing health and mental health needs
building social and community connections by engaging in activities that are meaningful to the resident
All program referrals are from CAA Placement committee meetings.
